Registration

When the physician gives you an order to receive outpatient testing or schedules you for testing, please report to the Registration Department which is located near the Emergency Department of Memorial Medical Center. The admissions office is open Monday through Friday, 7:30 a.m. - 4:00 p.m. Please arrive at least 30 minutes prior to your scheduled appointment. This time allows us to finish serving customers who arrive before you and to give you the time necessary to complete all required paperwork prior to your service.

Pre-registration expedites your check in process on your appointment day. There may be a few additional questions we need to clarify and we will ask you to sign a consent form. Please bring the physician's order and all insurance cards at the time of registration. If you have any questions, please contact our registration department at (361) 552-6713.

Physician Orders

When coming for outpatient testing you must bring your doctor's written order for treatment. The Federal Government states that a complete physician's order must include the following elements:

1. Patient's Full Name

2. One additional identifier (i.e., date of birth or social security number)

3. Service Ordered

4. Diagnosis or Signs and Symptoms

5. Physician Signature

6. Date of Order

Medicare requires us to verify medical necessity for certain outpatient testing. If the testing does not meet Medicare's guidelines, you will be asked to sign an Advanced Beneficiary Notice stating that you will be financially responsible for the service.

Radiologist Billing

The Radiologist at Memorial Medical Center is a private Physician specializing in the use of x-ray's, radioactive substance, and medical imaging used in diagnosis and treatment. It is the responsibility of the Radiologist to interpret the results of your examination or give the treatment in the Radiology Department and provide a written report and/or oral consultation with your physician.

The bill from the Radiologist is for the professional consultive services. The Radiologist charges are not included in the hospital bill. Please note that all Physicians participate in all insurance plans. Payment for service remains the responsibility of the patient.
