Home Health department is celebrating 20 years of serving our area.
The entire staff would like to thank the anniversary and would like
to do an add to thank the community and physicians etc for their continued
support.
For patients and
families alike, the challenges brought about by a permanent or short
term illness or disability can be overwhelming. To help you cope and
recover, Memorial Medical Center Home Health offers you skilled nursing
and professional health care in the comfort of your home. Designed
for those who require intermittent skilled care, the staff of the
Home Health program works directly with your attending physician to
coordinate a treatment plan designed especially for you. At all times,
our primary concern is to assure that you become as independent as
possible by minimizing the effects of your disability and illness.
Services
SKILLED
NURSING -The heart of Memorial Medical Centers Home
Health program is its skilled nursing staff. Working under the direction
of your physician, our trained RNs and LVNs give you quality care
and assist family members and caregivers in meeting the specialized
needs of the patient. Our skilled nursing staff is trained in a wide
range of specialities and services including blood pressure checks,
catheter care, catheter changes, diabetic teaching, dressing changes,
instruction on special diets, injections, IV therapy, ostomy care,
tracheostomy care, venipuncture and vital signs.
AIDE
SERVICES - To further assist in the comfort and recovery
of each patient, home health aides are available to supplement the
clinical care provided by our nursing staff. Aides are trained to
help with bathing, getting in and out of bed, skin care, prescribed
simple exercises and other general personal care designed to make
you more comfortable.
THERAPY
SERVICES
- Our goal is your recovery. Working with your physician, Home Health
will provide qualified and trained therapists.
MEDICAL
SOCIAL SERVICES - Each Memorial Medical Center Home Health
team member understands that recovery encompasses every aspect of
the patients life. Our social services staff member are there
for you and will help patients and their caregivers with the social
and emotional aspects of the patients care. They will assist
you in long-range planning and decision making, short-term therapy
options, financial assessment and planning, accessing community resources
and if necessary, resolving conflicts related to long-term illnesses.
LIFELINE
- Living on your own can be enjoyable and rewarding; however, sometimes
you may be concerned about the possibility of a bad fall, an accident
or sudden illness. For your security and comfort, Home Health is happy
to provide you with Lifeline.
Lifeline is a
personal response system which links you to 24-hour assistance at
the push of a button. A small, portable personal help button is worn
on a neck chain, wrist strap or clipped to clothing. If you need assistance,
simply push the button and help is just moments away.
Lifeline allows
you the freedom to move freely around your home or yard with the confidence
of knowing that if you need help, Lifeline will call for you.
Eligibility
Home health services are available to Medicare and Medicaid recipients
considered medically homebound and referred for care by a physician,
individuals covered by a private insurance plan that provides home
health benefits and individuals on a private pay basis. If you have
