Before clicking on "Yes" below to proceed with registration and consent for a Telehealth visit, you will need files available (saved to your computer or mobile device) to upload to the online patient registration form :
●front & back copies of your State-issued Identification (e.g. Drivers License) - This is required
●front & back copies of your Insurance Card - Required only if insured
