ID and Insurance

Based on your responses to the Covid-19 questionnaire, you qualify for Covid-19 testing.

Before clicking on "Yes" below to proceed with registration and consent, you will need files available (saved to your computer or mobile device) to upload to the online patient registration form :
●front & back copies of your State-issued Identification (e.g. Drivers License) - This is required
●front & back copies of your Insurance Card - Required only if insured

Click yes if you have your identification and insurance (if insured) files available and are ready to proceed with registering for the test.
